Summary

SUMMARY

 

The Nurse Case Manager serves as a critical clinical and administrative resource in the SDARNG's medical readiness mission. Responsible for managing complex healthcare cases, the Nurse Case Manager ensures that service members receive timely assessments, coordinated care, and follow-through necessary to restore or maintain deployment readiness. This role demands independent clinical judgment, deep familiarity with military medical systems, and the ability to lead care coordination efforts across multiple units and stakeholders. By proactively addressing health issues and guiding service members through the medical process, this position directly contributes to improving individual medical readiness and achieving the organization's target of 90–100% Fully Medically Ready status.

 

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

 

  • Provide a wide range of Medical and Dental Case Management and related Medical Administrative support services.
  • Work with all assigned soldiers in the SDARNG.
  • Ensure quality assurance and quality control for all case management and related medical administrative services, including daily review, spot checks, and remedial training for discrepancies in medical systems.
  • Provide oversight for Service Treatment Record (STR) maintenance and management and create standardized operating procedures for medical records maintenance.
  • Provide credentialing and privileging services for all SDARNG medical, dental, and behavioral health providers, ensuring they maintain current credentialing and privileging in accordance with AR 40-68.
  • Track the medical status of all SDARNG Soldiers in eCase and ensure thorough administrative charting with detail.
  • Chart Soldiers with cases in eCase.
  • Schedule profile exams/evaluations/Physical Health Assessments (PHA).
  • Coordinate Soldier treatment plans with appropriate medical facilities.
  • Maintain automated referrals indicated in systems such as MEDCHART, HealthCare Portal, eCase, and AVS.
  • Schedule, at a minimum, quarterly case management meetings with Major Command (MACOM) leadership to review cases, profile concerns, and extensions.
  • Attend PHA events as determined by the Deputy State Surgeon (DSS) to support state and individual MACOM needs.
  • Ensure that a case has been initiated and progress tracked at least once every 30 days for Soldiers referred by a provider for a medical issue.
  • Manage Soldier’s care from condition identification until the Soldier is either medically ready/returned to duty or administratively separated from the military.
  • Follow and ensure adherence to all regulations and policies applicable to IMR and deployment, including AR 40-501, and demonstrate knowledge of current regulations and policies.
  • Ensure all medical documentation received from the Soldier and/or their care facility is placed in an appropriate electronic record, as well as the Soldier’s STR.
  • Conduct review of Health Readiness Record (HRR) for all Soldiers who have an open case.
  • Maintain Soldier medical records in accordance with Army Regulation (AR) 40-66.
  • Provide records custodian or delegate with all relevant medical/dental documentation.
  • Obtain release of information from Soldiers.
  • Educate Soldiers on resources available to improve medical and dental readiness.
  • Maintain Health Insurance Portability and Accountability Act of 1996 (HIPAA) standards and guidelines and Privacy Act of 1974, ensuring strict confidentiality of service member records.
  • Review and track health assessment (PHA, SRP) lab results; contact Service Members (SM) and educate on lifestyle modifications.
  • Upload medical documents of case managed Soldiers into HRR within 5 business days of receipt.
  • Stamp all medical documents with date and initials for verification before uploading.
  • Index uploaded medical documents immediately after scanning documents into HRR.
  • Perform MODS entry for immunizations, designation of pregnancy, eye exams, and medical warning tags within 3 business days of notification.
  • Track all medical appointments or communication with Soldier and unit in eCase.
  • Run reports pre and post PHA/SRP and monitor MEDPROS to ensure all scheduled services are completed/updated and current medical status is reflected.
  • Coordinate methods for quality control, including monitoring process and system compliance, providing deficiency reports, and recommending courses of action.
  • Review referential and historical integrity of data warehouse information; provide quality assurance services for all information systems and databases.
  • Review data privacy policy and monitor compliance of data flows against data quality standards.
  • Develop, document, and maintain data quality goals and standards for medical operations, processes, and strategic plans.
  • Ensure requests for information are completed in accordance with regulation and policies within 30 days of receipt and processed through the Office of the State Surgeon.
  • Compile, enter, verify, and file medical documents and records in appropriate folders and databases, including scanning and indexing documents into medical and dental records.

SKILLS AND QUALIFICATIONS

 

  • Experienced in all aspects of case management.
  • Qualified in respective functional areas of case management.
  • Possess expertise as dictated by the “practice of nursing”.
  • Demonstrate knowledge in assessment, planning, implementation, coordination, monitoring, and evaluation of options and services to meet ARNG Soldiers’ health and human service needs.
  • Ability to apply systematic specialized knowledge and skills derived from biological, physical, and behavioral sciences to the care, treatment, counsel, and health teaching of individuals.
  • Knowledge of current Army, NGB, and state policies related to IMR and deployment.
  • Ability to implement procedures to identify, prevent, and ensure non-recurrence of defective services.
  • Ability to manage soldier care efficiently to meet deployment readiness goals.

 

EDUCATION & EXPERIENCE

 

  • Minimum of a Bachelor of Science in Nursing (BSN).
  • Minimum of 3–5 years minimum RN experience, ideally with:
    • 2+ years in case management or care coordination
    • Familiarity with military health systems or TRICARE is highly valuable

 

SUPERVISORY RESPONSIBILITY

No supervisory responsibilities are required for this role.

 WORK ENVIRONMENT

  • The primary duty station is Camp Rapid, 2823 West Main Street, Rapid City, SD, Bldg 420.
  • This is a dynamic, high-accountability role operating in a military clinical support environment.
  • The environment demands attention to military culture, adherence to security clearance requirements, and compliance with federal and state healthcare standards.
  • Case managers must work independently while embedded in a team-oriented setting, often juggling multiple complex cases with firm deadlines.
